The Clinical Social Worker is responsible for:

  • Providing social work services to all customer groups affected by illness/disability, often including lifestyle adjustment difficulties
  • Understanding utilization criteria and case management principles in a healthcare setting, as well as relevant community resources, insurance, regulatory, and legal system knowledge
  • The Clinical Social Worker reports and is accountable to the Director of Case Management and Social Work
  • Conducting Social Work assessment/counseling/education Services
  • Providing discharge planning services
  • Collaborating with inter-disciplinary team
  • Professional behavior and development
  • Documenting professional activity
  • Providing information/referral services for community resources
  • Demonstrating a commitment to patient safety
  • Acting as a Corporate citizen


The Clinical Social Worker must:

  • Have a minimum of a Master’s in Social Work
  • Have at least 3 to 5 years of social work experience in an acute care, IRF, or LTACH setting
  • Medical crisis intervention and knowledge of community resources or related experience is preferred
  • Be state-licensed as an LCSW in the state in which facility resides
